Use 8 1/2 x 11 card stock, score in half at 5 1/2 inches. Turn 90° and score at 2 1/8 and 6 3/8 inch.
Looking at the second photo, cut off the left and right sections to create the top fold half of the card base 
The dot embossing folder is unbranded from Temu. The Lacey circle die is unbranded from Temu. I will post the links in the comments.
The belly band is created with a 2 inch x 11 inches long strip of paper. Overlap and glue the circle over the join so it doesn’t show
